By signing below, I give my permission for my child to participate in the AWF DBCC Expedition Delta Outdoor Day Camps. I have read the policies and procedures set forth by the AWF DBCC and agree to abide by such policies. In the event that an accident does occur, I will not hold the AWF DBCC, its employees, volunteers, or Board of Directors responsible for any injuries. If emergency treatment or advice is deemed necessary by the Alabama Nature Center staff, I understand the above listed parent or guardian will be notified. If that individual cannot be reached, I authorize the AWF DBCC to arrange whatever emergency treatment is considered necessary.
